Sexual masochism is a form of paraphilia, but most people who have masochism do not meet the clinical criteria for paraphilic disorder, which suggest that the person's behavior, fantasies, or intense desires result in clinically significant discomfort or functional impairment. This condition must have been present for ≥ 6 months.

The prevalence of the paraphilic form of sexual masochism is unknown. However, one report from a telephone survey in Australia (2001 to 2002) found that 2.2% of men and 1.3% of women reported having been involved in sexual masochism and/or sadism during the previous 12 months.

Sadomasochistic fantasies and related sexual behavior by mutual agreement between partners is quite common. Masochistic behavior often takes the form of a ritual that is repeated over time. In most cases, humiliation and beatings are not entirely real; participants are aware that this is a game and avoid true humiliation and injury. However, some masochists desire more thrills over time, which can lead to serious injury or death.

Masochistic behavior may become the preferred or only way to obtain sexual satisfaction. People may use their masochistic fantasies on themselves, for example:

● Linking yourself

● Piercing your skin

● By giving yourself an electric shock

● Setting yourself on fire

Or they may find a partner - a sexual sadist. Partner activities include:

● Binding

● Blindfolding

● Flip flops

Flagellation (lashing)

● Humiliation in the form of urination or defecation on the body

● Forced to dress in clothes of the opposite sex

● Simulated rape
